Before an activity needing a HACCP plan, the permit applicant or holder submits one for approval where law requires it, where a variance is needed under 3-401.11(D)(4), 3-502.11 or 4-204.110(B), or where the regulator decides a method needs a variance; and before reduced oxygen packaging without a variance under 3-502.12, a HACCP plan is submitted to the regulator. The plan includes general information; the categories of TCS food covered (Pf); a flow diagram for each food or category identifying each step and the CCPs (Pf); ingredients, recipes, materials, equipment and control measures (Pf); a CCP summary for each food or category showing each CCP, its significant hazards, critical limits, monitoring method and frequency by the designated employee or person in charge, actions when limits are not met, how the person in charge verifies that employees follow procedures and monitor CCPs, and the records kept (Pf); supporting documents such as the employee training plan, blank record forms and any scientific data the regulator requires (Pf); and other information the regulator asks for.
