No one operates a food establishment without a valid permit from the regulatory authority (Pf). The applicant submits a written application on the regulator's form at least 30 calendar days before opening or before the current permit expires, is an owner or officer of the legal owner, complies with the Code, agrees to allow access and provide required information, and pays the fees. The application gives the applicant's and establishment's details, the ownership type, whether the establishment is mobile or stationary and temporary or permanent, which kinds of TCS food operations it runs (cook to order, advance preparation with discard, time as a public health control, multi-step preparation, off-site delivery, service to a highly susceptible population, non-TCS only, or prepackaged non-TCS only), the responsible person and their supervisor, the owners, officers and any resident agent, and a signed statement attesting accuracy and agreeing to comply and allow access to the establishment and to shellfish, water device and HACCP records.
