An eligible academic entity opting in must notify the EPA Regional Administrator in writing on the RCRA Subtitle C Site Identification Form (EPA Form 8700-12) for all laboratories it owns under the same EPA identification number (a VSQG without an ID notifies for all its on-site laboratories), completing at minimum the eleven listed fields (reason for submittal, site EPA ID except VSQGs, name, location, land type, NAICS codes, mailing address, contact, operator and legal owner, type of regulated activity, certification), and must keep a copy on file for as long as its laboratories are subject to the subpart. A teaching hospital or non-profit research institute not owned by a college or university must keep its formal written affiliation agreement with one on file. Withdrawal under 262.204 uses the same form and fields, returns the laboratories to 262.11 and 262.15 (or 262.14), and the withdrawal notice is kept for three years.
