A prescribed mine's operator must carry out a comprehensive, systematic safety assessment investigating and analysing major mining hazards so it understands every aspect of the related risk. It must consider those hazards both one at a time and in combination and use quantitative or qualitative methods, or both, suited to each hazard. The documentation must describe the methods; state each hazard's nature, the likelihood of it causing or contributing to harm to anyone on-site or off-site, and the severity of that harm; give reasons for the likelihood and severity conclusions; describe all controls considered and why each was implemented or rejected; and be readily comprehensible to its users. Under r 422 it must be kept ready for an inspector to examine on request.
