The operator of an MHF must carry out a comprehensive, systematic safety assessment of all potential major incidents and major incident hazards. It must investigate and analyse them so the operator understands in detail every aspect of the related health and safety risk, covering the nature of each hazard and incident, the likelihood of each hazard causing a major incident, the magnitude of an incident and severity of consequences for people on-site and off-site, and the range of risk controls considered. Hazards and incidents must be looked at cumulatively as well as one by one, using quantitative or qualitative methods, or both, suited to the hazard. The documentation must describe the methods, state the matters above, give reasons for the likelihood and consequence conclusions, and for controls considered state their viability and effectiveness and why each was chosen or rejected. Under r 370 the documentation must be kept ready for an inspector to examine on request.
