A self-employed person or employer must review, and revise where needed, the measures controlling risks from plant and its systems of work: before the plant is first used at a workplace; before any alteration to the plant or any change in how it is used or its systems of work, including moving it to a new location; when new or additional information about its hazards or risks becomes available; after any incident involving the plant or its systems of work that falls under Part 5 of the Act; when the controls do not adequately control the risks for any other reason; and when a health and safety representative asks for a review. A representative may make that request where the representative reasonably believes one of the other triggers exists, or that the self-employed person or employer has failed to review properly or to take account of a trigger in reviewing or revising.
