Additions to, modifications of or removal from the approved integrated circuit designer list (Supplement No. 6 to Part 740) or approved OSAT list (Supplement No. 7) are requested as advisory opinions by email, with the Supplement No. 4 information (identity, ownership and government or military ties, the advanced logic ICs and end uses, recordkeeping, a binding certification and an acknowledgement allowing on-site reviews); only entities not headquartered in Macau or D:5 that design, assemble, test or package ICs are considered; listed entities must request removal if dissolved or unable to comply and modification on a change of control or name; information is a continuing representation; records are kept under Part 762 and authorised designers must supply the information front-end fabricators report under 743.9 until delisted.
