Victoria Occupational Health and Safety Regulations 2017 r477: r 477 Apply for a replacement when an evidence of licence document is lost, stolen or destroyed

If the licence document goes missing, is stolen or is destroyed, the holder may seek a replacement from the Authority, using the form and manner the Authority requires and paying a fee of 1 fee unit. The Authority issues a replacement if satisfied the document was lost, stolen or destroyed; if not, it gives written reasons for refusing. Because the holder must be able to produce the document (r 476), a replacement should be sought promptly.
