Every person seeking a licence must put the following into the application: any proof of identity the Authority asks for; a photograph in the size and form the Authority sets, if one is required; a declaration on whether the applicant (and, for a company, each officer) has any past finding of guilt for an offence under relevant occupational health and safety legislation, with details of each finding; a declaration on whether any earlier licence or approval held by the applicant or an officer was suspended or cancelled under that legislation, with details; a declaration that, as far as the applicant knows, the contents are true; and, where the licence is wanted in a business name, that name plus the written proof of its registration the Authority requires.
