Where a service visit, inspection or test shows a system falling short of the applicable codes and adopted standards, or not being tested or maintained to them, a completed yellow label is fixed to the outside of the panel cover or in a permanent location to show that correction is needed; the licensee signs to certify that the listed conditions take the system out of compliance. Within five business days the licensee or firm gives the owner, occupant or representative and the local authority a written notice of the shortcomings, sent by post, email, fax or hand. The label stays on until the shortcomings are fixed and a service label certifying the fix is attached; a licensed employee or agent of a registered firm, or a regulator, may then remove it, and the local authority hears within five business days of its removal or revision. Yellow labels are about three inches square, removable adhesive, yellow with black lettering, and carry the do-not-remove inscription and the statement that the system fails the applicable codes and standards, in capitals of at least 10-point bold type, the firm's name, address, telephone number and certificate number, the date with the licensee's handwritten signature and licence number, and the list of non-compliant conditions.
