OSHA Safety and Health Regulations for Construction (29 CFR Part 1926) 1926.1431: 1926.1431 Hoisting personnel

Supplements the rest of the subpart whenever employees are hoisted. Hoisting people with covered equipment is prohibited unless the employer shows that conventional access (for example scaffolds, ladders, stairways, personnel hoists, aerial lifts or elevating work platforms) would pose greater hazard or cannot be used because of structural design or site conditions; this does not apply to Subpart R steel erection or routine shaft access under 1926.800. Employees must ride in a personnel platform meeting (e), except in drill shafts up to 8 feet diameter, pile driving, marine transfer and storage-tank, shaft and chimney work, each with its own rules. Set-up and equipment: level within one percent on footing a qualified person finds firm and stable; outriggers or stabilizers all extended equally and locked per the load chart; total load (platform, hook, load line, rigging) no more than half the rated capacity for the configuration and radius in use except in proof testing; brakes and secondary locks engaged when the occupied platform is stationary; boom angle indicator and boom hoist limiting device on variable angle booms (except derricks and articulating cranes), automatic overload protection on articulating cranes, jib angle indicator and jib hoist limiter on luffing jibs, boom length indication on telescoping booms, anti two-block device (pile driving handled under (p)), and controlled load lowering, all working with no alternative measures; no platform attached directly to a luffing jib. Platform: designed by a qualified person who knows structural design; stays within 10 degrees of level; minimises tipping; carries without failure its own weight plus a minimum of five times the maximum intended load; welded by certified welders; Subpart M guardrails with toeboard-to-midrail enclosure of solid material or mesh whose openings do not exceed 1/2 inch; inside grab rail; gates not swinging outward (unless infeasible on small platforms) with devices against accidental opening; standing headroom; overhead protection where falling objects exist; smooth edges; weight and rated capacity posted. Loading within capacity, only workers, tools and materials needed, secured and evenly distributed, occupants no more than designed or needed. Hooks closed and locked, alloy anchor shackles with bolt, nut and pin or secured screw pins, bridle legs to a master link, rigging and hooks supporting five times the intended load (ten times for rotation resistant rope slings), thimbles in sling eyes, and bridles used only for personnel hoisting. Trial lift with the unoccupied platform loaded to at least anticipated weight, before each shift and after a move or route change, with a competent person confirming devices, clearance, 50 percent limit and radius, then inspecting afterwards and checking test weight removal; before each lift, a few inches hoist and checks of ropes, twisting, centering and slack rope. Proof test to 125 percent of platform rated capacity at each jobsite and after repair or modification, held at least five minutes, and passed by a competent person before use. Work practices: slow controlled movement; occupants inside, not on rails, not pulling out of plumb; platform secured to the structure before entry or exit unless that is more hazardous; tag lines as needed; operator at controls (and in view) whenever the platform is occupied; qualified person using platform controls; wind over 20 mph or other dangerous conditions require a qualified person's decision on safety; direct communication; personal fall arrest attached inside the platform except over water (1926.106); no other load line lifts except pile driving, with a 500-pound limit for built-in platform winches; no travelling with personnel except on fixed rails or where no less hazardous way exists (never rubber-tired), on a prepared runway, within boom length, boom parallel, after a trial run; derricks never travel with personnel. A pre-lift meeting with operator, signal person, hoisted employees and task supervisor is held before the trial lift at each new location and for new employees. Hoisting personnel within 20 feet of lines up to 350 kV or 50 feet over 350 kV is banned except Subpart V work. Boatswain's chair and transfer-device variants (drill shafts, pile driving, marine transfer with Coast Guard flotation devices, tanks, shafts and chimneys) apply listed paragraphs, independent fall protection, chairs rated five times the load and one person at a time.
