Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition
Standard 1: Clinical Governance – Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition

Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition 1.11-1.12: Organisation-wide incident management and investigation systems support reporting by workforce and patients, involve them in review, feed back analyses, drive improvement and the risk register, and open disclosure follows the Australian Open Disclosure Framework

Incident systems must reach the whole organisation: staff helped to spot and report, patients and families given a way to raise concerns, staff and consumers drawn into reviews, findings fed back promptly to the board, staff and consumers, lessons turned into improvement, resulting risks entered on the register, and the system itself reviewed. Open disclosure must run on a program aligned to the national framework, with its effectiveness tracked and improved.
