Australia Model Work Health and Safety Regulations r371-373: rr 371 to 373 Health monitoring by an experienced registered medical practitioner, paid by the PCBU, with the required information

The PCBU ensures health monitoring is carried out by or under the supervision of a registered medical practitioner experienced in health monitoring, consulting the worker on the choice of practitioner; pays all its costs (shared equally between PCBUs with the same duty unless agreed otherwise); and gives the practitioner its name and address, the worker's name and date of birth, the work triggering monitoring and how long the worker has done it.
