What does 4.10-4.11 require?

Consent to access health information is sought using the consent wording that CALI settled with the GPs' professional college (the RACGP). Each time the consent is used the applicant is told, by phone, SMS, email or similar unless they prefer otherwise, why the information is sought and which party has been approached.
