Aged Care Serious Incident Response Scheme (SIRS) Provider Guidance rep-exclusions: Apply the exclusions only on the conditions the Rules set

An incident is not reportable where it results from a resident's refusal of services, or, in home or community settings, from a choice the individual communicated and the provider recorded in writing before the incident, provided it would otherwise only be neglect. Refusals by an informed person are recorded in the care plan with the circumstances, the reasons and the actions to address risk. The Commissioner may relieve notice where the same incident is repeatedly alleged because of a delusion.
