A death is an unexpected death where, for a resident of an approved residential care home, reasonable steps were not taken to prevent it, or where it resulted from the funded services delivered or from a failure to deliver them. The Commission's home services pages add examples such as a worker's mistake, care not delivered in line with assessed needs, or poorly managed care.
