Employers and self-employed persons must review, and where needed revise, their confined space controls after a Part 5 incident involving work in a confined space, whenever the controls are not adequate for any other reason, and when the HSR asks. The HSR may ask on reasonable belief that either of the first two triggers exists or that the review was not done properly or ignored those triggers (ss 21, 23, 24).
