As soon as practicable after the investigation, the head prepares and gives the QFCC a final report (maximum 100 penalty units) setting out the facts and circumstances, the findings including whether reportable conduct occurred, the reasons, copies of documents relied on, any referral to other entities, risk management and disciplinary action for a current worker, action to improve the entity's ability to identify, report and investigate, and reasons if no action is taken (s 37(1), (2)); the duty lapses where a sector regulator or the QFCC takes over and asks the entity to end its investigation (s 37(3)). The head provides further information the QFCC asks for after the final report (s 38); an entity exempted from the final report under s 39 still gives notice of findings, reasons and action (s 39(5)).
