The head of a reporting entity who becomes aware of a reportable allegation about, or reportable conviction of, a worker gives the QFCC written notice (an initial report) within 3 business days, or a longer agreed period, unless there is a reasonable excuse (maximum 100 penalty units); a reasonable belief that someone else has notified is a reasonable excuse (s 34(2), (4)). The initial report includes the allegation or conviction details, the worker's name, former names and date of birth if known, the head's name, whether the sector regulator or police have been contacted, the entity's contact details, and risk management and disciplinary action taken if the worker still works for the entity (s 35(1)).
