A documented appeals procedure describes how appeals are received, investigated and decided, how they are tracked and recorded with the actions taken, and how appropriate action is ensured, and a description of the process is publicly available; providers whose schemes use purely statistical evaluation seldom receive appeals and can treat appeals about evaluations as complaints. The provider acknowledges receipt, gives the appellant the outcome and progress reports, gathers the information needed to judge validity, is responsible for all decisions in the process, has the decision taken, or else reviewed and approved, by people with no involvement in the decision under appeal, and ensures investigation and decision involve no discriminatory action.
