Reports are clear, accurate, objective and comprehensive, covering the results of all participants (a summary where full data is impractical) with an indication of individual performance, and unless inapplicable or validly excluded they include: the provider's name and contact details; who authorised the report; which activities external providers performed where they affect the items or services; date of issue and status (preliminary, interim, final); unique identification of every component and a clear end; the extent of confidentiality; unique identification of the report and scheme; a description of the items including production and homogeneity and stability assessment; participant results with reported uncertainties; the statistical procedures; the statistical data and summaries, with assigned values, acceptable ranges and graphics; traceability and uncertainty of assigned values and how they were established; assigned values and summaries per method group where methods differ; the basis of the proficiency assessment standard deviation or other evaluation criterion; comments on performance; information on scheme design and implementation; advice on interpreting the statistics; and comments or recommendations from the round (continuous schemes may keep routine reports simpler with the rest in procedures or periodic summaries). Reports reach participants within planned timescales, preliminary results being allowed in sequential or perishable schemes; a policy governs how participants and customers may use reports; a new or amended report carries a unique identification, a reference to the report it replaces or amends and the reason; an amended report sent to a subset of participants is preceded by an analysis of the impact on the others; and any statement of participation or performance issued alongside the report is not misleading.
