EPA Risk Management Program (40 CFR Part 68) 68.60: § 68.60 Incident investigation

The owner or operator must investigate each incident that resulted in, or could reasonably have resulted in, a catastrophic release, starting as promptly as possible and no later than 48 hours afterwards, with a team including at least one person knowledgeable in the process and others with appropriate expertise; prepare a report with the incident date, the date the investigation began, a description, contributing factors and recommendations; promptly address and resolve findings and recommendations with documented resolutions; review the findings with all affected personnel; and keep reports for five years. For incidents meeting the 68.42 accident history criteria, the report must be completed within 12 months (unless the implementing agency approves an extension in writing) and must identify the initiating event, direct and indirect contributing factors and root causes determined with a recognised root cause analysis method.
