EPA Risk Management Program (40 CFR Part 68) 68.50: § 68.50 Hazard review

The owner or operator must review the hazards of the regulated substances, process and procedures, identifying the hazards; opportunities for equipment malfunction or human error that could cause a release; safeguards used or needed, including standby or emergency power, and ensuring that monitoring equipment for prevention and detection of releases has standby or backup power for continuous operation; steps to detect or monitor releases; natural hazards that could cause or worsen a release; and stationary source siting (placement of processes, equipment and buildings, hazards from nearby sources, and consequences from proximity to the public and public receptors). Checklists by knowledgeable persons may guide the review; for processes built to industry or government design standards, the review must inspect all equipment to determine whether it is designed, fabricated and operated to those standards. Results must be documented and problems resolved in a timely manner; the review is updated at least every five years and whenever a major change occurs, with all issues resolved before start-up of the changed process.
