The PCBU does not direct a worker to enter a confined space to work without a confined space entry permit completed in writing by a competent person that identifies the space, the people permitted to enter, the period of the work and the risk controls (based on the r 66 assessment, including controls for safe entry and the r 69 communication and monitoring system), with space to acknowledge completion; when the work is done all workers leave and the competent person completes the acknowledgement.
