Section 5: Implementation procedures of the OSHMS in the organisation (5.1 to 5.14) – ASEAN Guidelines for Occupational Safety and Health

ASEAN Guidelines for Occupational Safety and Health 5.8: Investigation of accidents, incidents, near misses and work-related disease to find root causes and prevent recurrence

What does 5.8 require?

The enterprise should investigate work-related accidents, incidents, near misses and diseases to find their causes and stop them happening again, concentrating on root causes rather than procedure. The recommended steps are: report the event to a designated person, give first aid and medical care, investigate, identify causes, report findings, plan corrective action, carry it out and evaluate whether it worked. Investigators should know accident causation and the work; in small businesses workers and supervisors may need to take part. The Annex 6 checklist structures the investigation and the written report.
