Critical defects are reported to the responsible entity before the technician leaves site, or as soon as possible, confirmed in writing within 24 hours of identification and reconfirmed each time found again. The remaining findings (lesser defects, missing-information items and late activities) go to the responsible entity inside seven days. Evidence of compliance takes the form of one or more yearly condition reports issued to the responsible entity, summarising the year's service records and stating: the site; the period covered; system identification and location; the applicable approved design Standard; outstanding defects and non-conformances; a summary of service records; a statement of condition; the responsible entity's name and address; and the service provider representative's name, signature and date and company. The report acknowledges missed activities and records any system that outstanding defects have left non-operational. It is intended to support state or territory regulatory compliance statements.
