A driver must be medically examined and certified as physically qualified by a medical examiner listed on the National Registry of Certified Medical Examiners, at least every 24 months or more frequently where the examiner so certifies, and must carry (non-CDL) or have on the State record (CDL) a current medical examiner's certificate (391.41, 391.43, 391.45). The 391.41(b) standards address loss of limbs, impairments of extremities, insulin-treated diabetes (now permitted under the 391.46 treating clinician assessment), cardiovascular, respiratory and neurological conditions, epilepsy, mental disorders, vision (20/40 in each eye with or without correction and 70-degree fields, or the 391.44 alternative vision standard), hearing (forced whisper at five feet or audiometric standard), use of Schedule I drugs or non-prescribed habit-forming drugs, and alcoholism. Drivers who do not meet the extremity standards may qualify with a skill performance evaluation certificate (391.49). The examiner uses the Medical Examination Report Form MCS-5875 and certificate MCS-5876, and the employer keeps the certificate (or the MVR showing certification for CDL drivers) in the qualification file (391.51).
