Specifies the content of the entry permit that documents compliance and authorises entry. The permit must identify: the space to be entered; the purpose of the entry; the date and authorized duration; the authorized entrants, by name or by some other method, for example a roster or tracking system, that lets the attendant tell quickly and accurately who is inside for the life of the permit (a reference on the permit to that roster or tracking method is acceptable); how an increase in atmospheric hazard levels will be detected if the ventilation system stops; each person currently serving as attendant, by name; the individual currently serving as entry supervisor, by name, plus the signature or initials of every supervisor who authorises entry; the hazards of the space; the measures used to isolate the space and eliminate or control its hazards before entry (for example lockout or tagging and purging, inerting, ventilating or flushing); the acceptable entry conditions; results of testing and monitoring under 1926.1204(e), with testers' names or initials and when each test was done; which rescue and emergency services are available to call and how to reach them, such as equipment to use and numbers to call; communication procedures between entrants and attendants during entry; the equipment provided for compliance, such as PPE, test instruments, communications, alarms and rescue gear; any other information needed for safety in the particular space; and any additional permits, such as hot work permits, issued for work in the space. An auditor checks each issued permit field by field against this list; a missing field means the permit does not meet the standard even if the underlying precautions were taken.
