OSHA Safety and Health Regulations for Construction (29 CFR Part 1926) 1926.1205: 1926.1205 Permitting process

Before authorising entry, each entry employer documents completion of the 1926.1204(c) safe-entry measures by preparing an entry permit. The entry supervisor named on the permit signs it before entry begins, which is the act that authorises entry. The completed permit is made available to all authorized entrants or their representatives at the time of entry, either posted at the entry portal or shown by another equally effective method, so entrants can confirm that preparations are done. A permit lasts no longer than the time needed to finish the task or job named on it under 1926.1206(b). The entry supervisor ends the entry and acts as follows: cancel the permit when the covered entry operations are finished; when a condition not allowed by the permit arises in or near the space but is temporary, does not change the space's configuration and creates no new hazard inside, suspend or cancel the permit and carry out a full reassessment before anyone reenters; and cancel the permit when any other prohibited condition arises in or near the space. The entry employer keeps every canceled permit for at least 1 year so it can be used in the program review under 1926.1204(n), and any problem met during an entry is noted on the relevant permit so the program can be revised appropriately.
