A hazard log is set up as the foundation of continuous safety risk management, tracks hazards to closure, is updated through the life cycle whenever an identified hazard changes or a new one is found, and includes or refers to its purpose, each hazard with the responsible entities and contributing functions or components, likely consequences and frequencies, the risk from each hazard, the risk acceptance principle and criteria applied, the measures taken to reduce or remove the risk, and exported safety constraints; an external hazard log (hazard record) extracts what other stakeholders need at the interfaces and for hazards no single stakeholder controls.
