Australia Model Code of Practice: Work Health and Safety Consultation, Cooperation and Coordination 3.6: 3.6 Keep brief records of consultation, and keep any records the Regulations require

Consultation need not be documented unless the Regulations require it (for example r 552 requires a major hazard facility's safety case outline to describe worker consultation), but records are good practice, help the risk process and make disputes less likely. Records can be brief: the safety matter, who was identified as affected, who took part, key issues, what was decided and why, who must act and by when, and when the action was completed. A health and safety committee should decide how its meetings are recorded.
