Australia Model Code of Practice: Managing the Risks of Plant in the Workplace 2.1d: Appendix C: Use the plant hazard checklist

Appendix C gives a checklist to help identify plant hazards, where a yes to any item shows the need for controls. It covers entanglement; crushing (material falling off, uncontrolled movement, tipping or rollover, collapse, contact during testing, inspection, operation, maintenance, cleaning or repair, being thrown or trapped); cutting, stabbing or puncturing; shearing; striking by moving or ejected objects; high pressure fluid; electrical hazards including live conductors, overload, damaged leads and lack of isolation; explosion; slips, trips and falls including from height; ergonomic hazards such as poorly designed seating or controls, high forces, repetition and vibration; hazard combinations such as unexpected start-up, over-run, control system failure, software errors and operator errors; psychosocial hazards (job demands, low control, poor support, traumatic events, remote or isolated work, poor environment, harmful behaviours); and other hazards including noise, lighting, confined spaces, unsuitable plant, hot or cold parts, hazardous chemicals, radiation and lack of operator competency.
