The PCBU must review and as necessary revise fall controls when a control does not minimise the risk so far as is reasonably practicable; before a change at the workplace likely to create a new or different risk the control may not handle; when a new relevant hazard or risk is identified; when consultation shows a review is needed; and when a health and safety representative asks for a review because they reasonably believe one of those circumstances affects their work group and the control has not been adequately reviewed (WHS Regulations r 38).
