The PCBU must identify all locations and tasks reasonably likely to cause injury from a fall, including access to and exit from the work area (WHS Regulations r 34). Inspect the workplace and talk with workers, perhaps with a checklist, looking at edges, holes, openings and excavations, surfaces (stability, fragility, load capacity, slipperiness, changes of surface, slope), changes of level, stability of structures, ground under scaffolds or platforms, crowded or cluttered work areas, access and exit, and places where grip may be lost. Give particular attention to work near unprotected edges, from elevating work platforms or ladders, on structures being built, demolished, inspected, repaired or cleaned, on or beside fragile surfaces such as cement sheet, rusted metal, fibreglass roofs and skylights, on unstable, sloping or slippery surfaces, and near holes, shafts and pits. Seek advice from manufacturers, suppliers, specialists and structural engineers as needed, and review incident records, near misses, complaints and industry information.
