The risk process must include ongoing monitoring and periodic review, at all stages, with responsibilities defined and actions recorded (for example planning, collecting and analysing data, recording outcomes and giving feedback). Risk is reduced by competent job planning, correct isolation and testing, fit-for-purpose safety equipment, tools, instruments and PPE, competent workers, warning notices and barriers. Work should be planned ahead where other services (gas, telecommunications, data, water, other circuits and energy sources) or other trades are present, and so that life-safety equipment such as emergency lighting or dialysis machines is not jeopardised.
