Aged Care Serious Incident Response Scheme (SIRS) Provider Guidance ims-record: Record the minimum details of every incident

For each incident the system requires a record of: the description including actual or reasonably expected harm and its consequences; whether it is reportable; when and where it happened or was alleged; when it was identified; the names and contact details of those involved and of witnesses; assessments made; actions taken; consultations with affected people; whether affected people received reports or findings; investigation details and outcomes; the recorder's name and contact details; and details of any police report. The Commission's prompt is who, what, where, when and how, plus related incidents, and records are updated as information arrives.
