The system can identify, record, assess, respond to and report on incidents; sets out procedures for each; requires support and assistance for affected people including advocates and language services; says how affected people are involved in resolving the incident; allocates roles for managing incidents and for notifying reportable incidents to the Commissioner; requires reportable incidents to be reported as the Rules say; states when the provider investigates causes, harm and contributing operational issues and the nature of that investigation; states when remedial action is required; and ensures the managing and prevention duties are followed.
