A residential care provider (outside specialist programs) must make the prescribed measurements and assessments, compile de-identified information from them and give the System Governor a quality indicators report for each quarter of the financial year within 21 days of its end, including exclusions and their reasons. The indicators are pressure injuries (staged, including those acquired outside the home, assessed once a quarter with consent during routine care), restrictive practices other than chemical restraint (a record review over an unpredictable three-day collection period), significant and consecutive unplanned weight loss (calibrated monthly weights with consent), falls and falls with major injury, medication management (polypharmacy of nine or more medications and antipsychotic use), activities of daily living, incontinence care, hospitalisation, workforce, the Consumer Experience Assessment, quality of life assessment, allied health, lifestyle officers and enrolled nursing. Quality indicator reports and collection records are kept for seven years (154-110).
