A nursing and transition care or residential care provider (outside MPSP and NATSIFACP) must give the System Governor, within 4 months after each 12-month reporting period, a report in the approved form signed by a governing body member for all members, stating whether the governing body believes the provider has complied with its conditions, obligations and requirements under the Act and, for each believed failure, what it was, why it happened and what rectification has been or will be taken; with information per home or branch on feedback and complaints, quality improvements and diversity and inclusion initiatives, board demographic representation (with consent), government status, and whether the section 157 membership conditions applied and were met.
