A person seeking an exemption must apply in writing, with the signature of the applicant or someone on its behalf and a date, giving the applicant's name and address; the person or class of persons covered; the workplace or undertaking if relevant; the provision, or the activity, process, substance or thing, concerned; why the exemption is wanted; the consultation done under r 540; and enough information for the Authority to decide under r 537 or r 538. One application may cover several persons, provisions or items, and the applicant must answer the Authority's written requests for further information.
