Victoria Occupational Health and Safety Act 2004 s38-3: s 38(3) Give the Authority a written incident record within 48 hours

Within 48 hours after the duty to notify arose, the duty holder must also lodge a written record of the incident with the Authority, using the form the Authority has approved in writing. Maximum penalty: 240 penalty units (individual) or 1200 penalty units (body corporate) (s 38(5)); the offence is indictable but may be heard summarily.
