Subpart A: General provisions (principal display panel, information panel, identity, ingredients, name and place of business, nutrition labeling, warnings, misbranding) – US FDA Food Labeling Regulations (21 CFR Part 101)

US FDA Food Labeling Regulations (21 CFR Part 101) 101.5: Name and place of business of the manufacturer, packer or distributor, with 'manufactured for' or 'distributed by' where not the manufacturer, and the street address rule (101.5)

The label must show conspicuously the manufacturer's, packer's or distributor's name and place of business, packer or distributor; where the firm named is not the actual manufacturer the name must be qualified by 'Manufactured for', 'Distributed by' or a similar phrase. The place of business is the street address, city, State and ZIP code, with the street address omitted only if the firm is listed in a current city or telephone directory. A corporation gives its actual corporate name (with 'Inc.' and the like optional) and may give the principal place of business rather than the actual manufacturing location.
