A high hazard establishment's duty holder prepares a comprehensive, site-specific safety report begun at the design stage, continually updated until operations start and maintained for the life of the establishment. It demonstrates a suitable and sufficiently documented plan showing that reliable built-in safety is incorporated in the design, construction, operation and maintenance of equipment and infrastructure, and that the major incident prevention policy, the process safety management system, the organisational and other measures to prevent major incidents and limit their consequences and the on-site emergency plan are applied; it also contains information on the off-site emergency plan. A proposed high hazard establishment submits a preliminary report while the establishment is being designed and a final report a reasonable time before operations begin; existing establishments send the report within 36 months of the Regulations; and the report is reviewed every five years, before any change to the establishment and whenever a change to the process safety management system could significantly affect major incident prevention or consequence limitation, and the revised report reaches the chief inspector within 60 days.
