The duty holder of a medium or high hazard establishment prepares and keeps a written major incident prevention policy on the Annexure C model, covering the construction and building of the establishment, changes to it and its safe operation, and has it in place within 36 months of the Regulations taking effect. The policy secures strong protection for employees and for the public, and includes at least its aims and objectives, the roles and responsibilities of management, process safety performance indicators, commitments to maintaining and continually improving the policy, the aims and objectives of the emergency plan and of the evacuation plan (speedy evacuation, roll-call after evacuation and plant shutdown), the reasons for revision, mandatory agreements and the process safety management system built on the Annexure D principles. The policy is reviewed every five years or when a change renders it inadequate, and an updated copy can be inspected by an inspector or the local government.
