The facility establishes procedures to investigate the sources and underlying causes of major OSH accidents (injuries, ill health, disease and work-related accidents) and informs the authorities concerned as appropriate. Every accident is investigated as soon as possible and closed with a comprehensive report; investigations are conducted by competent persons such as an OSH consultant or specialist, with worker participation as appropriate. Results and the immediate measures taken are notified to management and to the OSH committee or work team where one exists; reports from external investigating authorities are considered and acted on in the same way as internal investigations; corrective actions arising are implemented and followed up to prevent recurrence; the comprehensive report (date, findings, specific violations, follow-up actions) is documented in the documentation system and the competent authorities notified where possible; and the General Organization for Social Insurance is informed of work injuries and their causes.
