The employer reports every work accident or occupational illness resulting in a disabling condition, and every dangerous occurrence as defined in Rule 1053.02, to the Regional Labor Office in duplicate on the prescribed DOLE form, with a copy to the employee, on or before the twentieth day of the month after the accident or after the illness is established, estimating days of disability where the worker has not returned and correcting later. Deaths and permanent total disabilities are also notified within twenty-four hours by the fastest means and are investigated by the Regional Office within forty-eight hours. The employer keeps an accident or illness record open to authorised inspectors showing at least the date, the worker's name, sex, age and occupation, the assigned causes, the extent and nature of disability, the period of disability, property damage and its cost, and the record of notices and reports, and files an annual work accident and illness exposure data report. Disability is evaluated and performance measured (frequency and severity rates) by the definitions and scheduled charges in Rules 1051, 1055 and 1056. RA 11058 and D.O. 198-18 later restated reporting; the Rule is read with them.
