The verification report shall include at minimum a title, addressee, a statement of the responsible party's responsibility for the statement per the criteria, a statement of the verifier's responsibility to express an opinion, a description of the evidence-gathering procedures, the opinion, the date, the verifier's location and signature, a summary of the GHG statement, reference to the criteria and the scope.
