The verifier or validator shall maintain records of the engagement terms, the verification or validation plan, the evidence-gathering plan, who performed the evidence-gathering activities and when, the collected evidence, the requests for clarification, material misstatements and nonconformities with the conclusions reached, the communication with the responsible party on material misstatements, the conclusions and opinions, and the independent reviewer's name, review date and comments.
