ILO-OSH 2001 Guidelines on Occupational Safety and Health Management Systems
Chapter 3 Evaluation: performance monitoring, investigation, audit, management review (3.11 to 3.14) – ILO-OSH 2001 Guidelines on Occupational Safety and Health Management Systems

ILO-OSH 2001 Guidelines on Occupational Safety and Health Management Systems 3.12: Investigate the origin and underlying causes of injuries, ill health, diseases and incidents to find system failures, by competent persons with worker participation, documented, reported to the safety and health committee for recommendations, communicated for corrective action and fed into management review and continual improvement; implement the corrective action to prevent repetition; act on external investigators' reports in the same way

What does 3.12 require?

When work-related harm or an incident occurs, its origin and underlying causes are investigated to expose any failure in the management system, and the investigation is documented. Competent people conduct it with suitable participation of workers and their representatives. Findings go to the safety and health committee, where one exists, for its recommendations; findings and recommendations alike are passed to the appropriate people for corrective action, fed into management review and weighed for continual improvement. The resulting corrective action is implemented so that the harm or incident does not recur. Reports from outside investigators such as inspectorates and social insurance bodies are treated the same way as internal investigations, with due regard to confidentiality.
