The RMP must include a single registration form covering all regulated substances in covered processes with: source name, address, county, latitude and longitude with method and description; Dun and Bradstreet numbers of the source and corporate parent; owner or operator name, phone and address; the person or position with overall responsibility for RMP elements; the emergency contact's name, title, phone, 24-hour phone and email; for each covered process, each regulated substance above threshold with CAS number, maximum quantity in pounds to two significant digits, NAICS code and program level; the EPA identifier; number of full-time employees; whether subject to OSHA PSM (29 CFR 1910.119) and to 40 CFR part 355; any Title V permit number; the date and agency of the last government safety inspection; the RMP preparer; optional email, website, public inquiry phone, LEPC and OSHA VPP status; the type of and reason for changes (update, correction, de-registration, withdrawal); whether a public meeting was held after an RMP-reportable accident under 68.210(b); and the method and location of the notification that chemical hazard information is available to people within 6 miles under 68.210(d).
